Registration Instructions 

For Faculty/Staff
  1. On the login screen, click the green Login button and select the "OSU Employee Login" option
  2. Enter your Ohio State University username ("last name dot number").
  3. Enter your password
  4. You will be prompted to either sign up or log in to DUO, depending on if you have already registered
  5. Click the Login button
  6. LinktoHealth will display a screen that asks you to enter your:
    • Preferred Name
    • E-Mail Address
    • Preferred Phone Number
    • Secondary Phone Number
  7. After entering the above information, click the Register button.
For Spouse, Same-Sex Domestic Partner, or Dependent
  1. On the Login screen, click the New User Registration button 
  2. Enter the following information in the fields provided. This information will identify you in the LinktoHealth system.
    • Medical Insurance ID Number: This can be found on your medical insurance card. Enter the first 9 characters of your insurance ID (Ex. UF123456701 should be entered as UF1234567).
    • First Name: Must match the spelling used when you were enrolled for benefits.
    • Date of Birth: Use mm/dd/yyyy format, e.g., 03/25/1972.
  3. Click the Register button.
  4. LinktoHealth will display a screen that asks you to enter your:
    • Preferred Name
    • E-Mail Address
    • Preferred Phone Number
    • Secondary Phone Number
  5. After entering the above information, click the Register button.
  6. Create a Username and Password to access the LinktoHealth system.

Login Instructions

For Faculty/Staff
  1. Click the Login button on the Home screen.
  2. Click the Returning User button
  3. Enter your Ohio State University username ("last name dot number").
  4. Enter your password
  5. Click the Login button on the left center of the screen.
  6. Using DUO, obtain a two-factor log-in code to complete the login process
For Spouse, Same-Sex Domestic Partner, or Dependent
  1. Click the Login button on the Home screen.
  2. Click the Returning User button at the bottom left.
  3. Enter your Username. (If you previously checked the "Remember me" checkbox, then this will automatically appear.)
  4. Enter your password.
  5. Click the Login button.
  6. Using DUO, obtain a two-factor log-in code to complete the login process

If You Have Forgotten Your Username or Password

For Faculty/Staff
  1. Go to my.osu.edu, and follow the instructions there.
For Spouse, Same-Sex Domestic Partner, or Dependent
  1. Click the Login button on the Home screen.
  2. Click the Returning User button at the bottom left.
  3. Click the I forgot my username or password link.
  4. Enter the following information in the fields provided. This information will identify you in the LinktoHealth system.
    • Medical Insurance ID Number: This can be found on your medical insurance card. Enter the first 9 characters of your insurance ID (Ex. UF123456701 should be entered as UF1234567).
    • First Name: Must match the spelling used when you were enrolled for benefits.
    • Date of Birth: Use mm/dd/yyyy format, e.g., 03/25/1972.
  5. Click the Send my username or Reset my password button.
  6. Your existing username or new password will be sent to your e-mail address.
